  • Are you a veterinarian practicing in North America who diagnoses or treats any number of cases of canine heartworm disease each year? 
  • Do you want 1-hour of RACE approved CE and to be entered into a drawing to win an Annual Membership with the ODVC? (a $99.00 value) 
  • Are you willing to spend less than 15 minutes completing 3 surveys? 
  • The first 100 veterinarians who apply and meet the criteria will be enrolled in the online course.

I ascertain that I professionally prevent, diagnose, and/or treat canine heartworm disease in my practice (does not matter how many cases annually).*
I agree to complete a 1-hour RACE approved online training about using an evidence-based spectrum of care approach for preventing, diagnosing, and treating canine heartworm disease that was created by ODVC in collaboration with the American Heartworm Society.*
I agree to complete 3 surveys during the course: a pre-course, post-course, and a follow-up survey 6 months after completing the post-course survey that will be analyzed by the researchers at UC Davis Weill School of Veterinary Medicine*
I will provide my name, mailing address, email, phone number, and state/states I am licensed to practice to Open Door Veterinary Collective, but this information will be shielded from the researchers and not shared elsewhere.*
